Nomination for DISTRICT GOVERNOR, 2011-2012
All nominations shall include
the following documents:
1. Club's Resolution endorsing the nominee.
2. Complete Bio-Data of nominee with 2x2 photo.
3. Certificate of Physical Fitness of nominee.
4. Spouse's consent and support for nominee.
5. Acceptance of the nominee to the nomination.
Please submit 18 SETS of the above in hard copies to the District
Office not later than the announced deadline of January 20, 2009.
The undersigned, , PDG Bishop Cinense (Chairman of the District
Nominating Committee) and the Host, DGN Manny Yu, have scheduled the
CANDIDATE'S FORUM on February 21, 2009, Saturday, at the Fortune
Restaurant in Tarlac City. |
